To allow us to serve you better at your first appointment, be sure to bring:

• Insurance cards or forms provided to you by your insurance company

• A list of any current medications you may be taking

• Any recent x-rays if they are available

Please be prepared to fill out a brief medical and dental history to help us get to know you better. Current medical conditions are important to note, especially those that may necessitate premedication or special precautions during periodontal treatment. In fact, you might want to let us know about artificial joints or prosthetic heart valves when you schedule your appointment.
